Form for signing up your student to the grades 3-5 After School Program.
Dear Parent/Guardian of Student(s) in Grades 3-5,
The J.M.M.Y. Center is on Stonebridge Rd. in Georgia. The background checked volunteers at the center offer an after-school project club on Monday afternoons from 3pm to 5pm throughout the school year, except days when there is no school. This program is free of charge to families.
Students may ride the bus to the J.M.M.Y. Center provided the parent sends permission to the school. Parents must pick up their own child promptly at 5pm. Volunteers are scheduled until that time only.
If you plan to carpool with another family, the J.M.M.Y. Center must have written permission from the parent who is allowing their child to ride home with someone else. Parents must come inside to pick up their child.
If your child is signed up for project club but is sick, please call the J.M.M.Y. Center and let a volunteer know that they will not be there. Volunteers arrive around 2:50pm. The number is 802-355-2419.
If your child wishes to attend you may complete the permission form and return it to the school by Monday, September 16th.
Students will be given 2 unexcused absences but then may be excluded from future projects. This guideline is to keep slots open for children who are available to attend as well as for allowing volunteers to direct projects without needing to reteach skills or send home unfinished projects.
